Indonesia Again Faces Polio Outbreak: Get to Know the New Vaccine in the National Immunization Program

Sahabat Hermina, in the last few days, the world of children's health has been shocked again by the presence of 3 confirmed positive cases of polio, 2 cases in Central Java and 1 case in East Java, so it was declared a Polio outbreak again after the outbreak in Aceh in 2022. The child is known to have been immunized against Polio but was incomplete and the child was fully immunized against Polio but was malnourished. The World Health Organization or WHO classifies Polio disease in Indonesia asDiseases Outbreak News (DONs) or disease outbreaks that need to be watched out for.

In mid-2023, the Ministry of Health introduced a new type of vaccine which was added to the national immunization program which the government is starting to implement. There are 4 types of new vaccines, including the Conjugated Pneumococcal Vaccine (PCV) which will provide effective protection to prevent pneumonia (lung inflammation) caused by pneumococcal bacteria and is also expected to be an effort to prevent stunting, the Human Papilloma Virus (HPV) vaccine to prevent cervical cancer, Rotavirus vaccine (RV) to protect children from diarrhea attacks caused by rotavirus, and a second dose of Inactivated Poliovirus Vaccine (IPV) vaccine to strengthen children's protection from Polio.

IPV immunization itself functions to protect a person from polio virus infection. The second dose of IPV immunization will begin to be given in 2022 in 3 provinces, namely DKI Jakarta, West Java and Banten. And in 2023 it will be expanded nationally. According to the Ministry of Health's guidelines, IPV is given twice, the first dose is given at the age of 4 months and the second dose is given at the age of 9 months. Considering that in the last few days the Polio outbreak has occurred, it is very important to pay attention to the type of vaccine and the latest immunization schedule that must be fulfilled according to government recommendations.

Other immunizations such as PCV and HPV are also carried out in stages. In Indonesia, around 14.5% of deaths in babies and 5% of deaths in toddlers every year are due to pneumonia. For this reason, PCV immunization will begin to be included in the national program starting in 2022, given twice when children are 2 and 4 months old and once when children are 12-24 months old. HPV immunization has also been given to elementary school students in grades 5 and 6 and in 2023 it will be introduced as a national program.

Furthermore, the last immunization included in the national program is rotavirus immunization which functions to prevent diarrhea in children. To provide high and even protection, it is recommended to give it 3 times, namely when the baby is 2, 3 and 4 months old. Giving the vaccination itself is easier, namely via mouth drops, not by injection.

In accordance with Health Law Number 36 of 2009 which states that every child has the right to receive immunization in accordance with the provisions to prevent the occurrence of diseases that can be avoided through immunization.

So, we have high hopes for all parents to try to provide support to their children in scheduling complete routine immunizations according to the provisions. Thus, coverage of complete routine immunization can increase so that the goal of immunization, namely the formation of individual immunity, group immunity and cross-group immunity, can be achieved. Stay healthy.

Polio Alert!!! Get to know the symptoms, causes, treatment and prevention

Polio is a disease caused by a contagious viral infection. Exposure to this virus can trigger nerve injury which has a very high risk of causing paralysis, difficulty breathing, and can even lead to death.

Even though children are very susceptible to this disease, this does not mean that adults are not at risk of being exposed to polio. Taking appropriate precautions can be an effective measure to avoid exposure to the virus that causes polio.

Causes of Polio

This disease is caused by the polio virus. Usually, transmission can occur through direct contact or consuming water and food that has been contaminated with feces containing the polio virus. Even though they have no symptoms, polio sufferers can still transmit the polio virus to other people.

Polio Risk Factors

People who have a very high risk of developing this disease are:

People who live in remote areas have difficulty accessing clean running water, especially for toilets
HIV Positive Pregnant Women
Children who are not vaccinated

For people who have never been vaccinated, the risk of contracting this disease will be higher if:

Travel to an area where there has been a recent polio outbreak
Living with or caring for someone with polio
Work with virus specimens
Have had a tonsillectomy

Polio Symptoms

The symptoms of polio are experienced differently by each sufferer. In fact, 95 to 99 percent of polio sufferers do not experience symptoms. Here are some symptoms of polio to watch out for:

Polio Symptoms of Non-Paralytic Type Polio

Fever
Painful Swallowing
Headache
Vomit
Weak
Meningitis

Polio Symptoms of Paralysis Type Polio

Loss of Reflexes
Feet become droopy
Paralysis that occurs suddenly can be temporary or permanent

Polio Diagnosis

The diagnosis of polio itself is made based on anamnesis, physical examination, and can be assisted by supporting examinations. In the history, the doctor will look for symptoms that appear, then through a physical examination the doctor will look for signs of disease such as stiff neck and abnormalities in reflexes.

Supporting examinations through throat swabs, stool examination and central nervous system fluid analysis can also be carried out to look for the presence of the polio virus.

Polio Treatment

See a doctor as soon as possible. Currently there is no medicine that can treat polio. However, this disease can be overcome by carrying out several treatments according to doctor's recommendations, such as

Giving medication reduces pain
Antispamodic medication to relax muscles
Antibiotics
Physiotherapy

Polio Prevention

Prevention of polio is by vaccination. Polio vaccination itself has been discovered since 1957 and is one of the most effective prevention efforts. Vaccinations are given 3x and supplemented with 1x booster. Vaccinations need to be given to children at the ages of 2 months, 4 months, 6-18 months and boosters between the ages of 4-6 years.

The Importance of Immunization for Children

Immunization goals

prevent dangerous infectious diseases before they spread in the community.

How do vaccines work?

Immunization àof the body forms immunity àThe immune system will form specific immunity against certain diseases.

In the future, if the child is infected with the disease, the child will not get sick because the immune system will immediately react quickly against the disease.

Why should children be immunized?

Immunization is a safe and effective way to prevent diseases that are widely circulating in the community. The benefits of immunization are enormous when compared to the very small risk of vaccine side effects.
If enough people in a community are immunized, it won't take long for the infection to spread from person to person.
Children who have been immunized if they get sick are generally mild.

Why do children need to get so many immunizations?

Several immunizations are needed in the early years of a child's life to prevent dangerous diseases. The more diseases that can be prevented, the higher the chances of a child living with a better quality of growth and development.

The baby's immune system is immature. The baby's immune system cannot work as well as in older children or adults, so immunizations need to be repeated.

Is immunization safe?

Some children may experience mild vaccine side effects, occurring within 1-2 days after receiving immunization and recovering quickly without having to receive medication.

The most common side effects are redness, swelling, pain at the injection site, and may be accompanied by low-grade fever or irritability.

How to handle:

Give more to drink
Don't wear thick clothes
If necessary, paracetamol can be given to reduce fever.

It should be remembered that the currently available vaccines are very safe compared to if the child has to suffer from a disease that can be prevented by immunization.

How long will immunization provide immunity?

In general, immunization will produce immunity after 2 weeks of administration. This means that prevention of the disease is not immediately obtained after the child has received immunization. Some immunizations need to be repeated several times to get long-term prevention.

What are the consequences if immunization is late?

The immunization schedule has been prepared taking into account the exact time when a child must be protected against dangerous infectious diseases. If it is too late to give immunizations at the proper age, children will be vulnerable to disease transmission.

For immunizations that must be given several times, the distance between the two immunizations is 4-8 weeks. If it is too far, the immunity that is formed is not optimal.

Do late immunizations have to be repeated?

If you realize that the immunization is late, immediately come to the health facility and ask for the missed immunization.

Hermina's friend,

Child immunization is the administration of vaccines to children to prevent transmission of certain diseases. Exposed to Measles? How does the measles virus cause disease?

Measles is an acute viral infection caused by Paramyxovirus. Transmission of the disease happens by airborne. As an infectious disease, measles may cause serious health problems for babies, pregnant women, and immunocompromised people.

In developing countries, measles is still a serious health threat for people from all age; especially to children under five years old. Examples of serious complications which may occur including:

Diarrhea with profuse vomiting that may cause dehydration.
Infection of lower respiratory tract (pneumonia) which is the complication with the highest fatality rate.
Infection of the middle ear which may cause hearing impairment.
Vitamin A deficiency which may cause eye disorder.
Infection of the brain that may cause death.
Subacute Sclerosis Panencephalitis (SSPE) - a neurological disorder that affect the central nervous system - caused by measles infection during childhood.

To pregnant women, measles may cause labor before due date so that the infant will have low birth weight.

Symptoms of measles infection are high fever, coughing, red & inflamed eyes, and red blotchy skin rashes that may cover every part of the body. These symptoms usually occur after 10 to 12 days after measles virus infection.

After 10 to 12 days of infection, the first symptom that appears is a high fever (body temperature > 38.0° C) that may persists for 2 to 5 days. During this fever, other symptoms such as coughing and red & inflamed eye may also occur. Clinician may also find Koplik's Spots which is a diagnostic feature of measles. Koplik's Spots is a small white spot on a reddened background that can be found on the inside of the cheeks, but it may only be found during one or two days before skin rash occurs & usually disappears in 12 to 18 hours.

During the fourth or fifth day of fever; the fever will reach its peak; and will be followed with the appearance of red blotchy skin rashes. These skin rashes will appear first on the area behind the ears; then they will spread to the neck, face, then to every part of the body surface. These skin rashes may persist for three days, then they will subside gradually in order of the onset of these skin rashes.

Measles without complication can be treated as an outpatient care. Medications will be prescribed according to each case's symptoms. To facilitate faster recovery; patients are advised to have sufficient meal & drinks and ample rest. Children with measles may also take a bath or shower to clean themselves. For children with measles, they will also be given Vitamin A suplement with doses according to their ages. To prevent transmission to other people; patients with measles are advised to undergo quarantined for at least for days since skin rashes occur or until their symptoms subside.

For measles with complications; patients are advised to be admitted in a hospital to receive further treatment. Examples of high-alert signs form measles with complications are:

Loss of consciousness and seizures
Coughing with difficulty breathing
Dehydration caused by frequent diarrhea
Weight loss and malnutrition
Ear disorder due to infection
Dry eye wih vision impairment

To prevent measles, children are recommended to get measles vaccine. The vaccination can start to be administered to children around nine months of age. If your child has mild symptoms of kidney disease, consult to your doctor immediately to get relevant advice regarding the possibility of early preventive treatment.

Have Difficulty Urinating? Understanding Urinary Retention

Urinary retention is the loss or reduced ability to empty the bladder completely. This situation is part of the lower urinary tract / lower urinary tract symptoms (LUTS).

In general, urinary retention is divided into 2 conditions, which are acute urinary retention and chronic urinary retention

Acute Urinary Retention

Usually accompanied by pain
Can't urinate at all
Full bladder
Happened suddenly
Including emergencies in urology

2. Chronic Urinary Retention

Can be painless
Can still urinate, but not facile
Inability to completely empty the bladder (incomplete)
Difficulty starting to urinate (hesitancy)
Not life threatening, but can cause complications

The causes of urinary retention can be distinguished based on their etiology:

Supravesika: motor and sensory nerve innervation disorders due to neurological diseases such as stroke, HNP etc
Vesica: bladder detrusor muscle weakness, antimuscarinic/ anticholinergic drugs (low bladder pressure)
Infravesika: bladder outlet obstruction (BPH, urethral stricture, meatal stenosis, urethral edema, urethral masses, urethral stones, pelvic organ prolapse), increased urethral resistance, detrusor-sphincter dyssynergia (DSD),

Immediate treatment in cases of acute urinary retention is to decompress the bladder using urethral catheterization, suprapubic puncture or suprapubic catheterization (cystostomy). In most cases, clinicians prefer to insert a urethral catheter and only perform a suprapubic puncture or cystostomy if the urethral catheter failed. Suprapubic catheterization is frequently associated with the incidence of haematuria and catheter obstruction than urinary catheters.

In a study by Horgan et al, it was reported that the incidence of urinary tract infection (UTI) was lower with suprapubic catheterization than with urethral catheterization (18% vs. 40%; p<0.05) and less incidence of urethral stricture at 3-year follow-up ( 0% vs 17%; p<0.01) (Fitz; yoon). Insertion of a suprapubic catheter also carries several risks, a 2.5% chance of intestinal injury and a 1.8% risk of 30-day mortality. Therefore, The British Association of Urological Surgeons recommends insertion of an imaging-assisted suprapubic catheter (USG).

Before consulting with an Urologist, it is better to record the complaints what you are experiencing. Remembering what you were doing before the complaint was felt, whether you had an injury or not. In addition, collect a complete medical history, including any medical history or medical history. This is useful for doctors to diagnose what diseases you are experiencing.

Proper Nutrition to Overcome High Blood Pressure

High blood pressure or hypertension is an increase in blood pressure that affects the function and structure of blood vessels. Hypertension can cause heart failure and coronary heart disease. One of the principles of therapy in hypertension is consuming proper nutrition. The type of diet recommended for hypertension is the dietary approach to stop hypertension or known by the abbreviation DASH.

The DASH diet is recommended by various health professionals, including the American Heart Association (AHA), to lower blood pressure and the incidence of heart disease. On the DASH diet, patients are advised to reduce the consumption of foods high in sodium, while the consumption of food sources of potassium, magnesium and calcium needs to be increased to help lower blood pressure. According to the AHA, the current recommendation for sodium consumption should be less than 2,300 mg or 5 g of table salt per day. For populations that are sensitive to the use of sodium, it is recommended that consumption does not exceed 1500 mg per day. According to the Centers for Disease Control and Prevention, the population that is said to be sensitive is people with hypertension and the elderly. Reducing sodium intake has been shown to significantly improve health. Several studies have shown that reducing sodium intake is also associated with preventing hypertension, lowering blood pressure, and preventing cardiovascular disease.

In general, in people who will undergo the DASH diet, it is recommended to increase the consumption of vegetables, fruit, low-fat dairy products, whole grains, chicken, fish, and nuts. Conversely, the consumption of foods that contain high fat and sugar, red meat, and soda should be avoided. It is recommended that you consume 4-5 servings of fruits and vegetables a day. In addition, consumption of omega 3 essential fatty acids containing EPA and DHA of 1 g/day also has benefits for heart health. To achieve this recommendation, it is recommended to consume fatty fish at least 2 times a week.

In addition to a healthy diet, maintaining an ideal body weight is also recommended to prevent or control blood pressure. Physical activity can help you lose weight, improve blood sugar control, lower cardiovascular risk factors, and improve fitness. Physical activity is any movement that increases energy expenditure. In people over the age of 18, it is recommended to do 150 minutes of moderate-intensity physical activity per week or 75 minutes per week of active aerobic movement or a combination of both. Pause between workouts of no more than 2 consecutive days.

If hermina's friends need the right guide for the DASH Diet, please consult a health specialist at the Hermina Podomoro Hospital Nutrition Specialist.
