Terms and conditions :

Photocopy of KTP & KK of the patient's parents
Cleft lip : Minimum age 3 months (cleft lip) and weight 5 Kg
Cleft Palate : Minimum age 12 months and minimum body weight 10kg
Healthy condition, no fever, cough, runny nose and pale.
